This agreement is between GLOW DH1 Limited ("GLOW") and the undersigned customer ("Customer"). By signing or confirming this agreement digitally via the QR code provided in-store, the Customer acknowledges their full consent and understanding of the terms outlined below.

1. Personal Information and Health Declaration

The Customer has provided the following information as required by GLOW: confirmation of pregnancy status, contact details (including address, skin type, and emergency contact information), and marketing preferences.

The Customer has also confirmed whether they currently suffer from or have experienced any of the following:

• Ill effects from normal sunbathing
• Conditions such as epilepsy, giddiness or fainting, headaches or migraines, or heart conditions
• Blood pressure issues, hypertension, or prickly heat
• Cold sores, allergies, or skin ulcers
• A family history of skin cancer or a large number of moles and/or freckles
• Currently under medical supervision
• Prescribed or taking any form of drug, antibiotic, or tranquilliser
• Diabetic, pregnant, or hypersensitive to sunlight
• Recent surgery within the last six weeks
• A medical condition worsened by sunlight exposure
• A history of sunburn, particularly in childhood

2. Acknowledgment of Sunbed Usage Instructions

The Customer acknowledges that they have been instructed on the safe operation of the sunbed, including the use of the emergency stop mechanism.

3. Confirmation of Suitability for Tanning Treatments

The Customer confirms that they have read and fully understand the information provided by GLOW regarding tanning treatments, and consider themselves suitable for tanning treatments based on the information disclosed.

4. Protective Eyewear

The Customer confirms that they have collected the free protective eyewear provided at the counter and understand its importance in ensuring safety during tanning sessions.

5. Block Course Expiry

The Customer understands and agrees that all block courses will automatically expire one (1) year from the date of purchase.

6. Take at Your Own Risk

The Customer acknowledges that tanning treatments, including the use of sunbeds, carry certain risks. These risks include but are not limited to:

• Skin damage, including sunburn or premature aging
• Allergic reactions or irritation
• Increased risk of skin conditions, including skin cancer
• Exacerbation of pre-existing medical conditions

By signing or confirming this agreement, the Customer accepts full responsibility for their decision to undergo tanning treatments and agrees to do so entirely at their own risk. GLOW shall not be held liable for any adverse effects resulting from the use of its facilities, provided that all instructions and guidance were followed as outlined.

7. GDPR Compliance and Data Protection

GLOW is committed to protecting your personal data and ensuring compliance with the General Data Protection Regulation (GDPR).

• Data Usage: The personal and health information provided will be used solely to ensure the safe provision of tanning treatments and to manage your account effectively.
• Storage and Retention: Your information will be securely stored and retained for the duration necessary to fulfill the purpose of its collection, in compliance with legal and regulatory requirements.
• Data Sharing: Your information will not be shared with third parties without your explicit consent unless required by law.

By signing or confirming this agreement digitally, you consent to the collection, use, and storage of your personal data in accordance with our GDPR policy.

8. Legal and Disclaimer

The Customer acknowledges the following:

• The information provided is accurate and truthful to the best of their knowledge.
• They understand that knowingly providing false information may result in legal action pursued by GLOW.
• They have been informed of the risks associated with tanning treatments and have voluntarily chosen to proceed.
